An Independent Private Practice

This is a strictly private, self‑pay psychiatric practice.

Dr. Miller does not participate in insurance networks and does not submit claims to third‑party payers.

 

The independent structure preserves:

  • Complete clinical autonomy

  • Protection from insurance‑driven treatment limitations

  • Extended, unhurried evaluations

  • Elevated confidentiality standards

  • Continuity of care under direct physician oversight

A detailed superbill may be provided should patients elect to independently pursue potential out‑of‑network reimbursement.

Clinical decisions within the practice are guided solely by medical judgment and established standards of care.

Pathways of Care

The practice offers two distinct and carefully defined models:

I. Consultation Psychiatry (Traditional Appoinment‑Based Care)
II. Executive Concierge Psychiatry (Membership‑Based Psychiatric Oversight)

Tier 1 and Tier 2

Each pathway reflects a distinct level of structure and physician accessibility.

CONSULTATION PSYCHIATRY 

Traditional Appointment‑Based Care

 

A structured outpatient model for individuals seeking comprehensive psychiatric evaluation and ongoing management within a traditional framework.

A $75  appointment  reservation fee is required no less than 48 hours in advance to secure the initial evaluation. The reservation fee is applied toward the total evaluation fee.   Credit card payments incur a 3.5 percent processing fee.

In‑person evaluations are preferred in order to allow for optimal clinical assessment.

Telemedicine may be considered for individuals residing more than 50 miles from the office.

 

Initial evaluations must be canceled at least 48 hours in advance. Cancellations within the required timeframe will result in a refund of the reservation fee. Appointments canceled outside the required notice period are subject to forfeiture of the reservation fee.

For the in‑office Initial Evaluation, patients are asked to arrive early to complete the required documentation. Late arrivals may require rescheduling in order to preserve the integrity of the schedule.

EXECUTIVE CONCIERGE PSYCHIATRY

Membership‑Based Psychiatric Oversight

Tier 1 and Tier 2

Executive Membership is a limited‑availability model designed for individuals who require enhanced physician accessibility, discretion, and a more individualized structure of care.

Enrollment is intentionally limited to preserve access standards and clinical responsiveness

Membership is governed by a separate written agreement outlining the scope of services, access standards, and financial structure.

A $150  appointment  reservation fee is required no less than 72 hours in advance to secure the initial evaluation. The reservation fee is applied toward the total evaluation fee.  

Initial evaluations must be canceled at least 72 hours in advance. Cancellations within the required timeframe will result in a refund of the reservation fee.  Appointments canceled outside the required notice period are subject to forfeiture of the reservation fee.

Concierge membership is offered following review of clinical appropriateness and availability.
