Dr. Venera R. Miller offers a limited, membership‑based psychiatric program for individuals whose professional roles require sustained judgment, composure, and resilience under pressure.

This concierge‑level model is structured for those who operate in environments where decisions carry weight, schedules shift unexpectedly, and discretion is essential.

Both in‑person and telepsychiatry services are available. Psychiatry is uniquely suited to telemedicine, offering flexibility and confidentiality while maintaining the same level of clinical depth and attentiveness. For in‑person visits, a private waiting area is provided to ensure discretion and comfort.

Care is available across Washington, DC, Maryland, and Northern Virginia.

Some professional lives do not fit within conventional medical scheduling.

​The Executive Program is intentionally structured to provide:

All tiers provide comprehensive outpatient psychiatric care. The difference lies in containment capacity and adaptability when circumstances shift.

Executive Tier I

Priority Access Psychiatric Care

Designed for high-functioning professionals who benefit from structured, predictable psychiatric care with enhanced access within clearly defined professional boundaries.

Includes:

• 120-minute comprehensive initial evaluation

• Up to 12 scheduled 45-minute follow‑up appointments annually

• All scheduled visits within the annual allotment are included in the membership fee and are not billed separately.

• Expedited scheduling within standard calendar flow

• Secure direct messaging access

• Same‑ or next‑business‑day response during business hours

• Ongoing medication optimization and monitoring

• Coordination with therapists, primary care physicians, or other treating professionals (with consent)

• Telehealth or in‑person flexibility

• Structured treatment planning with longitudinal review

• This tier provides enhanced continuity while maintaining a contained and predictable structure.

 

Appropriate for individuals seeking enhanced continuity and access beyond the traditional model, without requiring intensive oversight.

Tier I is best suited for individuals who are clinically stable and do not require frequent schedule adjustments or intensified oversight between visits.

Executive Tier II

Advanced Executive Clinical Oversight & Priority Access

Designed for individuals in senior leadership or high-stakes roles where clinical stability, discretion, and rapid responsiveness are essential.

Includes:

• Extended comprehensive evaluation (120–150 minutes as clinically indicated)

• Up to 14 flexible-length follow‑up appointments annually (45–60 minutes)

• All scheduled visits within the annual allotment are included in the membership fee and are not billed separately.

• Priority scheduling accommodations when clinically indicated

• Direct physician access with prioritized response during business hours

• Up to 2 urgent same‑week appointments annually when necessary

• Proactive monitoring during periods of instability, transition, or increased stress

• Coordination with medical, legal, or executive teams (with consent)

• Travel continuity planning and crisis mitigation strategy

• High-level psychopharmacologic management with longitudinal oversight

 

This tier provides a closely managed, strategically structured psychiatric relationship for individuals whose professional performance is mission-critical.

Tier II is appropriate for individuals navigating periods of instability, complex medication management, or professional environments where rapid clinical adjustment may be necessary.

The distinction from Tier I is not additional appointments, but greater flexibility, containment capacity, and strategic responsiveness when circumstances shift.

For individuals requiring highly individualized, high-intensity outpatient psychiatric oversight, a limited Tier III structure is available on a case-by-case basis. Scope, availability parameters, and financial structure are individually defined.

As part of the Executive Concierge Tier I and II memberships, the Practice will utilize Spruce Health, a HIPAA-compliant secure communication platform, for direct text messaging between the Member and the Practice.

Membership Structure

 

Executive Membership is an annual or semiannual bundled care model designed to provide enhanced continuity, structured access, and reserved clinical availability.

Membership fees include the defined services within each tier, including scheduled appointments and associated clinical oversight as outlined in the tier description.

By intentionally limiting panel size, this model allows for longer appointments, responsive communication within defined parameters, and thoughtful longitudinal treatment planning.

This program does not provide 24/7 psychiatric coverage, inpatient services, or emergency crisis care. In urgent or emergent situations, patients are directed to appropriate emergency resources.

Unused services do not roll over beyond the membership term.
