Our practice is dedicated to providing thoughtful, individualized psychiatric care in a confidential and professional setting. The following outlines how we structure our services.

This practice offers two models of care:
– Structured consultation-based outpatient services
– Membership-based concierge services (Tier 1 and Tier 2)

Policies outlined on this page apply to standard consultation services.

Concierge membership services are governed by a separate written membership agreement.

Privacy & Confidentiality

This practice is committed to protecting your health information in accordance with:

  • HIPAA Privacy and Security Rules (45 CFR §§ 160, 164) 

  • 42 CFR Part 2 (substance use disorder records) 

  • Maryland's Confidentiality of Medical Records Act (MCMRA) (MD code § 4-301)

  • District of Columbia Medical Confidentiality Statute (DC Code § 7–1303.02)

  • Commonwealth of Virginia Medical Confidentiality Statute (VA. Code § 32.1–127.03) 

Confidentiality may be limited in situations involving imminent risk of harm to self or others, suspected abuse or neglect, court order, or other disclosures required by law.

Fees & Insurance

Dr. Venera Miller is a non‑participating provider with all insurance plans.

All services are self-pay. Documentation for out‑of‑network reimbursement can be provided upon request. Patients are responsible for verifying their insurance benefits directly with their carrier.

Payment is required prior to each appointment. A 3.5 percent processing fee applies to credit card transactions. Returned payments are subject to an administrative fee.

Failure to provide payment at the time of service may require rescheduling. Outstanding balances may result in the temporary suspension of services.

Appointments

 

Initial Evaluation
An appointment reservation fee is required to reserve the initial evaluation and is applied to the evaluation fee. Cancellations require 48 hours’ notice.

Follow-Up Visits
Ongoing appointments are scheduled according to clinical need. A minimum of 24 hours’ notice is required for cancellations.

Missed or late-canceled appointment fees will be applied.

Office hours are Monday–Thursday 9:00 am–4:30 pm and Friday 9:00 am–1:30 pm.

Patients arriving more than 20 minutes late for an initial evaluation may require rescheduling.

Late arrival for follow-up visits may result in a shortened session.

Repeated missed appointments may result in discharge from the practice.

Appointment reminders are provided as a courtesy; patients remain responsible for attending or canceling within the required notice period.

Medication Management

When medication is part of treatment, prescribing is conducted carefully and in accordance with state and federal regulations.

Regular follow-up is required for ongoing prescriptions.
Monitoring procedures may be implemented when clinically appropriate.
Early refills and replacement of lost controlled prescriptions are not provided.
These standards are in place to ensure safe and responsible care.

Refill requests must be submitted at least 48–72 hours in advance and are processed during regular business hours only.

Pharmacies are not authorized to provide automatic refills.

If a patient has not been seen within a clinically appropriate timeframe, a follow-up appointment will be required prior to renewal.

Continuity of Care 

Patients who have not been seen for more than one year may be considered inactive and may require re-evaluation before resuming care. Re-enrollment is determined on a case-by-case basis.

Telepsychiatry

Secure virtual visits are available through a HIPAA-compliant platform. Patients receive a private link prior to their scheduled appointment.

Communication

For non-urgent matters, patients may leave a voicemail or send a secure email. Messages are reviewed during business hours and responses may take 24–72 hours depending on clinical volume. While reasonable safeguards are in place, electronic communication carries inherent privacy risks. Email and voicemail must never be used for emergencies.

Emergencies

If you are experiencing a medical or psychiatric emergency, call 911, contact 988, or proceed to the nearest emergency department.

This practice provides outpatient care and does not offer emergency services.

Medical Records

Patients have the right to obtain copies of their medical records upon written request in accordance with applicable state and federal law. Reasonable administrative fees may apply as permitted by law.

Legal & Administrative Requests

Court appearances, depositions, or legal testimony require a separate written agreement and advance payment. Clinical appointments may be rescheduled if legal obligations require physician attendance.

Termination of Care

Either party may terminate the therapeutic relationship. When termination occurs for non-emergent reasons, reasonable notice and referral resources will be provided consistent with professional standards.
